These are Tony Kosoko Physiotherapy's own terms and conditions. They were written to be signed on paper at the clinic, so the signature block has been left off here. Agreeing to them in person is still what counts.
Bupa insurance policy holders
We are happy to invoice Bupa on your behalf although this will not affect your liability as being responsible for the payment of your fees in full. You will need to provide us with your Bupa membership number and a valid authorisation code. Please be aware that Bupa do not pay for appointments cancelled without 24 hours' notice.
Please note that your private medical insurance may not cover all your fees and expenses. Certain treatments and items may not be covered by your insurer, or there may be an excess payable. We will require that you pay any balance if your insurance company does not cover the full cost of treatment. If we receive notification from your insurer that you have an excess on your policy, your funds have been exhausted, or if they refuse to pay any outstanding fees in full for any reason, you will be liable for any unpaid fees in full.
Other insurance providers
If you are not covered by Bupa you will be required to pay at the time of your appointment. We are happy to provide you with a receipt to reclaim this charge from your insurer.
Referrals
If we are referring you on to an expert opinion we will send a referral letter to this third party. You will then be responsible for arranging your appointments and payments. We undertake no liability with respect to the third party.
Consent to treatment
Many medical and therapeutic procedures and treatments can be confusing for patients. Our practitioners make every effort to ensure you are fully aware and understand all that is being said. If you are uncertain at any stage, please make us aware so we can explain further.
Tony Kosoko Physiotherapy operates in accordance with the Data Protection Act 2018. Your medical records will be stored and accessed by authorised personnel only.
By signing our terms and conditions you are consenting to us handling your personal data, including your health records, and to the uses referred to above. Please ask us for more information with regards to data protection, or see our Privacy Policy with regards to data protection, your health records and related rights, if you are unsure.
Our liability
We do not accept liability for the loss of or damage to patients' personal possessions. We do not accept liability for death or personal injuries unless proven to be caused by the negligent act or omission of Tony Kosoko Physiotherapy or its employees. Your statutory rights are not affected.
Cancellation policy
We require at least 24 hours' notice to cancel an appointment. Appointments cancelled with less than 24 hours' notice will be charged in full.
Using this website
Nothing on this website is medical advice, and neither the pages nor the chat assistant are a substitute for an assessment. Sending an enquiry through this website does not create a booking: an appointment is confirmed only when we reply to you and agree a time.
