Privacy Policy

Here at Tony Kosoko Physiotherapy, we take the protection of your data and your privacy seriously and this page provides information on what personal data we collect, how and why we collect it, how we store it and for how long we keep it. This page also explains how we look after your data and protects your rights in accordance with the relevant data protection legislation.

What personal information do we hold about you?

Name, address, date of birth, phone contact details, email, GP details, NHS number. Record keeping in accordance with Chartered Society of Physiotherapy guidelines.

Why do we collect your information?

In accordance with our governing body and to send appointment confirmations and reminder emails, to contact you regarding appointment changes. To allow us to write medical letters and send onto you, your GP or Consultant, only if you have given permission.

Who has access to your data at the clinic?

Physiotherapists and admin staff within the clinic.

Who do we share your data with outside the clinic?

We will only share the data with your express permission, for example letters to GPs and Consultants, and for legal purposes such as solicitors acting for you in a legal case.

How long do we keep your information?

In line with our governing bodies, the Health and Care Professions Council (HCPC) and the Chartered Society of Physiotherapy (CSP), medical records form a legal record of treatment and therefore must be retained safely and securely in accordance with the General Data Protection Regulation 2018.

As per CSP guidance for private practices, we store records in accordance with NHS guidelines and as such dispose of adult medical records eight years after the conclusion of treatment or death; after the 25th birthday of the patient if they were under the age of 18 when treatment concluded; and after the 26th birthday if they were under the age of 17 when treatment concluded.

General

Tony Kosoko Physiotherapy does not sell your data to third parties or other organisations. From time to time, Tony Kosoko Physiotherapy will use your personal data to conduct analysis that will provide useful information.

Using this website

The following describes what happens when you use this website, as distinct from the clinical records described above.

The enquiry form

When you send an enquiry through this website I collect your name, email address, phone number, the service and location you chose, what you tell me about your problem, whether you have insurance, and any preferences you give. It is used only to reply to you and arrange an appointment. If you do not go on to become a patient, the enquiry is kept only as long as needed to deal with it and is then deleted. If you do become a patient, it becomes part of your clinical record and is kept for the periods set out above.

Anything you write in the enquiry form about your problem is health information, so please give only what you are comfortable sending by email. There is no need to describe your symptoms in detail at this stage.

The chat assistant

The chat assistant on this website answers general questions about fees, opening times, locations, insurance and booking. It cannot give medical advice and does not ask for personal details. Nobody monitors it. Tony is not notified of anything you type and does not see it, so please do not use the chat to describe symptoms or to ask for urgent help.

Nothing you type in the chat is stored. Your messages are sent to Anthropic, the provider of the artificial intelligence that writes the replies, purely to produce an answer, and they are not used to train its models. Once you close the chat window the conversation is gone.

Cookies and what is stored on your device

This website sets no cookies at all. Nothing is stored on your device, there is no advertising or tracking of any kind, and no analytics.

The maps on the locations page are the one exception, and only if you ask for them. They load from Google only when you press "Show map", and Google may then set cookies of its own under its own terms. Until you press that button, nothing is requested from Google and no map is loaded.

Your rights

You have the right to ask for a copy of the information held about you, to have inaccurate information corrected, to ask for erasure or restriction in some circumstances, to object to certain processing, and to data portability. Some of these rights are limited where the information forms part of a clinical record that must be retained. To exercise any of them, contact us using the details below.

Complaints about data protection

If you have taken steps to have a concern or complaint about Tony Kosoko Physiotherapy's handling of data resolved but are still not satisfied, you have a right to lodge a complaint with the Information Commissioner's Office (ICO), who are the relevant regulator for data privacy and protection matters. Updates to this policy

The clinic sections were last updated 24 May 2022 and are reviewed as necessary. If there are significant changes in the processing of your personal data which will impact you, we will act to inform you directly.
